When you turn 18, many things change
You get to vote, get a driver’s license, and manage your own finances. Another change is that, around the age of 18, you will move from pediatric to adult healthcare, which may feel both exciting and a bit nerve-wracking.
STEPSTONES is a supportive program for those living with chronic illness who are transitioning into adulthood with all that it entails. The goal of the program is for both you and your guardian to feel secure during the transition.
A suitable starting point for the STEPSTONES program is usually between 14-16 years old.

How does STEPSTONES work?
The core of the program consists of individual meetings with a transition coordinator, meaning without parents. A transition coordinator is a specialist nurse with specific knowledge of young people and the transition between pediatric and adult healthcare. Together, you’ll develop a plan for your transition to adult healthcare.
Often, things other than your illness affect life. That’s why you’ll also discuss other topics, such as school, career choices, diet and exercise, friendships, alcohol and drugs, and sexuality. Or anything else you wish to talk about. The nurse you meet with, of course, has confidentiality.
What does the STEPSTONES program include?
- Three individual sessions with a specialist nurse, your transition coordinator, approximately once a year. Your coordinator guides you throughout the transition and is available if you have questions.
- A personalized plan to prepare you for your transition to adult healthcare. You’ll create this plan together at the first meeting.
- Advice and support regarding the transition and your illness, but the sessions can also cover other important aspects of your life.
- Group sessions for young people and parents/guardians. You’ll meet others in similar situations, the adult healthcare team, and older teens who have already gone through the transition.
- A final meeting with your coordinator and staff from both pediatric and adult healthcare.
- Support for parents/guardians as needed.
